Home Plus
Supports vulnerable older people who are in hospital awaiting discharge and have concerns about returning home.
- A volunteer advocate will give support to help address these concerns in order to facilitate a positive return to the community.
- The advocate can offer information and advice about relevent community services which people may wish to engage in.
- They will also visit people in their own homes to see how they're getting on after a hospital discharge.
